Hi Thivanka, thanks for posting. You are already able to customize the form in that you can change all of the text. However, you aren't able to add additional fields.

Andrew Rasmussen: I think a few of us need forms that are more customizable (ability to add dates requested by, additional fields, etc). The fields are somewhat limited as they currently stand.
